2. Eligibility

Open to legal residents of the 50 U.S. states & D.C., 18+ at time of entry. Employees, officers, directors, contractors of Sponsor and its affiliates/subsidiaries/agents, and their immediate family/household members, are not eligible. Void where restricted.

3. Promotion Period

Begins October 1, 2025 at 8:00 AM MST and ends October 31, 2025 at 11:59 PM MST. Entries received outside this window are void.

4. How to Enter (Instagram Only)

During the Promotion Period, entrants must:

1. Follow @simonmedimaging and @simonone_bysimonmed on Instagram.
2. Like this giveaway post and @simonmedimaging last three posts on Instagram.
3. Comment on this giveaway post and tag at least one (1) friend.

Additional Entries:

• Each additional unique friend tagged in separate comments = 1 additional entry.
• Share this giveaway post to your Instagram Story and tag @simonmedimaging = 2 additional entries (limit one Story)

5. Prize; Medical Suitability

• One (1) prize: simonONE Whole-Body MRI Scan
• Prize covers the scan only; travel, incidentals, and any additional medical follow-up are not included.
• Medical Appropriateness: If the winner is not an appropriate candidate for receiving a whole-body MRI scan, Sponsor may, at its discretion, offer a reasonably comparable alternative (e.g., a different screening or SimonMed service credit of equal or lesser value).
• Expiration: Prize must be scheduled and completed within 12 months of winner notification unless otherwise stated by Sponsor.
• Taxes: Winner is responsible for all federal, state, and local taxes and any costs not expressly stated as provided by Sponsor. No cash substitution, transfer, or assignment permitted, except by Sponsor if the prize becomes unavailable.

6. Winner Selection & Notification

• On November 1, 2025, Sponsor will select one (1) potential winner in a random drawing from all eligible entries.
• Sponsor will contact the potential winner via Instagram Direct Message from @simonmedimaging.
• Potential winner must respond within 72 hours with requested information (full name, email, and phone number) to confirm eligibility and claim the prize.
• A SimonMed representative will then reach out directly to schedule the MRI scan.
• If potential winner fails to respond or provide required information within the timeframe, an alternate may be selected.

7. HIPAA / Privacy

No protected health information is required to enter. Any health information shared for publicity is voluntary and requires a separate HIPAA authorization. Personal data collected will be used to administer the Giveaway and as described in Sponsor’s Privacy Policy.

9. General Conditions

Sponsor reserves the right to disqualify any entry that is incomplete, ineligible, inaccurate, or non-compliant with these Rules or Instagram policies, or that appears to be generated by bots/fraud. Sponsor may cancel, suspend, or modify the Giveaway if technical issues, fraud, or events beyond its control impair administration, security, fairness, or proper play. In such event, Sponsor may select a winner from eligible, non-suspect entries received prior to action or as otherwise deemed fair.

10. Release; Limitation of Liability

By participating, entrants agree to release and hold harmless Sponsor, Instagram, and their respective parents, affiliates, officers, directors, employees, and agents from any and all liability for injuries, losses, or damages of any kind arising in connection with participation or prize acceptance/use. IN NO EVENT SHALL SPONSOR BE LIABLE FOR PUNITIVE, INCIDENTAL, CONSEQUENTIAL, OR SPECIAL DAMAGES.

This promotion is in no way sponsored, endorsed, administered by, or associated with Instagram. By entering, you understand you are providing your information to Sponsor, not Instagram, and you release Instagram from any responsibility related to this promotion.
